What Is

Support Coordination? 

We use a Person-Centered Team approach as Support Coordination is a Case Management Component of New Jersey’s Supports Program, and Community Care Program for people with intellectual, mental health and developmental disabilities. This service is designed to respect the hopes and dreams of individuals and their families by facilitating a unique and individualized planning process that enables consumers to make informed choices about eligible services and supports. NJ Support Coordination is funded by Medicaid Fee-For-Service in the comprehensive waiver. As support coordinators, our role is divided into these areas:

  1. Individual Discovery
  2. Plan Development
  3. Coordination of Services
  4. Monitoring Progress

Since our Support Coordination Agency can approve our own support plans, we ensure an efficient and swift implementation. Our coordinators:

  • Connect all individuals with community resources within their counties, ensuring true integration within the community with the assistance and protection of services offered through DDD
  • Link all individuals we support with resources that celebrate their culture and religious beliefs

  • Are multilingual and can assist individuals who speak the following languages: English, Spanish, French, and African dialects of Nigeria, Ghana, etc.

  • Understand the difference between acting as a resource, which is part of our role, and speaking for the individual or family, which is not part of our role

  • Are highly experienced and educated with a minimum BA requirement and certification in person-centered thinking/planning

  • Receive continuous cross training in crisis management, community building and integration, and employment
